EFT form problems

Does anyone have a good eft form that they have their clients fill out? I have one that we have been using for a couple of years but it makes me think there may be something better out there. In our forms it states that your are signing for a minimum of so many months at which point it will continue unless you cancel. But I am tired of hearing "well I didn't read that I want my money back!"
Also does anyone have a form that they have people fill out when they do cancel?? I'm also tired of hearing "well i canceled three months ago and your still charging me I want my money back!!" Please someone help me!! My email is

I personally just use the one that is supplied by suntouch and make a line in my release form that says the terms, so they are double signing once on the release and once on the EFt form with the amounts. I print the same EFT form with the deactivate date and have them sign it as well.

also - give them a copy of the contract that they are signing - that way they can stand there and read it - sign it - and have a sopy to keep - so they can never sya " i didn't read it, i didn't know that, blah, blah, blah

I have it lined out that they have to come in and sign a form to freeze or cancel their membership by the 20th of the month before or a final debit will be made.

On my forms, I made a spot for them to initial lines of particular importance, like if they come in to cancel, I make them sign the line that says "I understand that if I cancel after the 20th, one final debit will be made on the 1st." or something like that...

When they freeze, I made a line for them to initial that says, "I understand that the next payment will automatically start back on __/__/__" and we fill in the date.



I have different forms for each thing... when they cancel, freeze, or modify their info. Sign, sign, sign. LOL.

me too - makes life so much easier and then they can't turn it around on us and play stupid -

calls for responsibility on their part - what program are you using wendyjo?
we have all our contracts set up within helios - makes it nice for us.

I find no matter what they sign and initial, people never read what they sign, so I Alwyas give a quick verbal synopsis of what the terms are they are signing....I don't want anyone to feel like they were tricked into something....explain the details and make sure they understand
