Again, this was created for a large corporation. Feel free to change it to suit your needs.ABSENTEEISM/TARDINESS POLICY You are expected to report to work on time, on a regular basis. Unnecessary absenteeism and tardiness is expensive, disruptive and places an unfair burden on other employees and your supervisor. Unsatisfactory attendance will result in discharge. Unnecessary absenteeism and tardiness will impact any promotion considerations. If you are going to be absent for any reason, telephone your supervisor (not the answering service of the switchboard operator) between 7:00 a.m. and 8:30 a.m. Explain why you are going to be absent or late when you expect to return to work. NOTE: It is your responsibility to ensure that proper notification is given. Asking another employee, friend or relative to give this notification is not acceptable, except under emergency circumstances. Failure to provide the proper notification will result in your being charged with an unexcused absence. Repeated absenteeism, repeated tardiness, or unexcused absenteeism will subject an employee to discharge. Any time you have missed work for any reason, you must notify the Personnel Director of your absenteeism.ABSENTEEISM Under the Company's policy an incident of absence consists of two or more hours in any workday or series of working days during which you are absent from work. This includes absences on days when you were scheduled to work overtime.EXCEPTIONS Employees will not be considered absent when they are not at work because of: Ÿ A paid holiday (except when scheduled to work on the holiday);Ÿ A regular scheduled vacation;Ÿ Authorized sick day; Ÿ A leave of absence authorized prior to the absence; Ÿ Being required to serve jury duty; Ÿ Funeral leave; Ÿ Suffering a work-related injury which prevents working; Ÿ A leave of absence under the Family and Medical Leave Act; andŸ Other reasons approved in advance by management. Employees who fail to properly notify the company of their absence for three (3) consecutive workdays will be considered to have voluntarily terminated their employment with the company. Employees who provide false reasons for an absence are subject to disciplinary action, up to and including discharge. The Company also reserves the right to request reasonable documentation for the reasons given for an excused absence.TARDINESS An employee is tardy when he is late to work by more than ten minutes during any scheduled work period, including returning from lunch or other approved time off during the workday. An employee's tardiness is unexcused unless prior approval from the employee’s supervisor is obtained.EXCESSIVE ABSENTEEISM AND TARDINESS Each employee's general attendance record will be examined whenever an incident of absence or tardiness is posted on the employee's time card. The employee's record will be reviewed for the previous twelve (12) months. The number of absences and tardies will be combined in determining the proper discipline. Each absence will be considered an incident. Two tardies will also be an incident for purposes of this policy. Incidents will be accumulated and disciplinary action taken pursuant to the following steps. The Company, however, reserves the right to take appropriate disciplinary action, up to and including discharge, for any incident based on the specific facts and circumstances. STEP ONE Two (2) incidents occurring during the twelve (12) month period measured from the most recent occurrence will result in a first written warning. STEP TWO Four (4) incidents occurring during the prior twelve (12) month period measured from the most recent occurrence will result in a second written warning. STEP THREE Six (6) incidents occurring during the prior twelve (12) month period measured from the most recent occurrence will result in a last chance written warning

In light of the fact that busy season is right around the corner and many of us will be hiring new faces soon, I am bumping this topic for those who have not seen it.
