TANNING BED CONSENT FORMS

I have a very basic form for my clients to sign, Im wondering if I should update/change my form to something more formal..... Advice on where I could get a form online to download and use? We operate 1 salon in WV. Thanks!!!!

If you send me your email address I will send you a copy of our client form. We rewrote it to cover all the bases.....

WE have a great form we got from Sun Ergoline. Iv been told though legally these forms dont mean squat. I have like 10,000 of them filled out. I am running out of places to keep them. If I did need to find one it would be difficult. We used to keep in a file cabinet. I have started just stacking them on some back shelves. If I needed to find one I guess I could go by date.

OUr tanners have to fill out a health history and release just like the people who are working out, then tanners have another information /release form to fill out
