Spray Tan Questionnaire - tanTALK

I am working on adjusting the questionnaire that I have new clients fill out before I tan them, do they tan in the sun, have they spray tanned before, are you pregnant ect What do you ask and what have found to be the most helpful?

Any allergies? Any medications? Do they have a hard time getting a tan from the sun? (because that could translate that they might have a hard time with a sunless tan also)

Are they allergic to fruit or nuts since some solutions contain both and if you use latex gloves to spray be careful for people that have latex allergies! I have barely gotten started and have had more potential clients with allergies needing a spot test prior to a full body spray... The questionnaire is really important!
