Homebeach, if you have a website or a Myspace page, You should post your consent forms on their so the customer can print it off at home and bring it in with them. Thats what I've done.

3. For best results wait eight hours after airbrushing before showering.

5. All the components of the airbrush tanning solution have been safely used in cosmetics and nutritional products for decades and have been proven safe for the skin. If you have had adverse effects utilizing self tanning products or moisturizers in the past, you may experience similar effects with the airbrush system. Please discuss this with the technician before proceeding.

6. We recommend that you hold your breath during facial spraying. We make this recommendation because most individuals would prefer not to breath the mist. The solutions is not considered harmful in the minimal amounts that may be inhaled in a normal spray tanning session.

7. If you have a history of asthma or other respiratory or medical conditional that could be aggravated by the use of the airbrush systems, please consult a physician before proceeding. Please apprise your tanning technician of any past allergic reactions to DHA products or other medical concerns.
I, the undersigned understand and agree to comply with all instructions. I have been properly instructed prior to my airbrush session. I am using these services at my own risk. I hereby authorize and direct employees or agents of the salon to perform such tanning procedures as may be deemed necessary or advisable, and have provided them with the above information required. I acknowledge that the results of airbrush tanning do vary and that no guarantees of specific results are offered or implied. The salon and their agents or employees are not liable for any injury to persons or property or the loss or theft of any personal property and hold them harlmess from any liability involved in the use of the tanning process.

I DO or DO NOT (please circle one) give permission for Oasis Tanning Spa to use before and after photos for advertising purposes and understand that they will be used only for promoting healthy tanning alternatives and not for entertainment purposes.

Why would they wear goggles? They would have major racoon eyes. You MUST Spray over their closed eyes or it would look ridiculou. Also I'm not sure why they would need lip balm. Hmmmm?

Nose plugs would only be used if they have asthma or breathing issues imo.

FDA has not decided if dha inhaled is harmful. Nose plugs are a must for us.

It was my understanding that DHA is FDA approved for spray tanning. I do have nose plugs but no one wants to use them and I dont force anyone to use them either.

Also when I took my classes and certification I was told Nose plugs are not a MUST.

You are 100% wrong unless they gave approval this past week. Nose plugs are a must and to many salon owners tout spray tanning as being safe and they contradict themselves by not following the guidelines.
