release forms
Can someone lead me in the direction of getting a release form for my clients for the spray tanning. I just bought the portable one from Cal Tan, and have been waiting forever for the emailed release form. First he said that I didn't need one, and so I asked him if he was going to be my lawyer later. So he said he'd email me one, but haven't heard yet!!!!!! Can someone send me or lead me to a place for one. Thanks. Just new to the whole tanning thing. Got sprayed by my friend, so thought I'd put it in my salon (have one in my home in a small town of 900 people) Getting excited to try it.....but need release form first. thanks for the info

Found this old post. Not sure if it helps. I don't do sunless, so I don't have one.
www.tanningtraining.com has a great section called sunless buisness kit and in that section is a great release form just for sunless!! |

how about,
I am going to do my best to make you look good, but it doesn't work for everybody. No guarantees. And, yes, If you want it sprayed, I have to see it, but I won't touch it. If you still want me to spray you, sign here and you are over 18, go in there and get naked, I'll be in in a minute. If you are under 18, get your Mom to sign here and take her in there with you. |

You don't want it to be as crass as what I wrote, but if it doesn't say pretty much the same thing (only nicer) you are wasting your time. I have both regular tanning and spray tanning addressed in my release form. I do not want anything in that form in any way to be construed as a guarantee of results or as implying that tanning or spray tanning will not harm you in any way. I will tell the customer what I think. I tan and my 16 and 18 year old daughters tan, but if you believe your dermatologist who told you that you would die from skin cancer in 5 years if you tan, hey, maybe you shouldn't tan. Never had someone decide not to tan because the release form scared them out of tannning. |
