Seperate client liability form for sunless tanning...

Do you offer the same client forms for both Mystic and UV tanning or do you offer seperate forms for each service?

www.tanningtraining.com has a great section called sunless buisness kit and in that section is a great release form just for sunless!!

i use a pre made consent form for both provided by one of our suppliers. im really intrested to see what type of forms other salons are using because the forms that we use seem like info cards rather than consent forms

The ding-dong that owns a Jamaican gift/craft shop here, and installed a Mystic has no consent/waivers for their clients to sign.............. I'm tempted to go over there with my white tuxedo and get a paint job!? Maybe I can expand after that law suit?
