Im looking all over the web to find a disclosure for my new clients to sign , just to cover my self in case of an allergic reaction to DHA. Anyone have any ideas???

Some airbrush solutions are hypoallergenic and that eliminates that problem... also anyone who wants a skin test we have them come in and we spray their butt. This way it wont show.

You can do a search at http://www.sunless.com/phorum/list.php?f=3 which is an Airbrush Tanning Message Board. There is a lot of useful information on this board and if you can't find a disclosure there you could make a post and I'm sure someone would be willing to help you out.

by disclosure do you mean a "waiver form" that clients fill out before a tan? Sorry, just to make sure, I would be more than happy to send you an email, with an attachment of the waiver form I use, the only thing missing is a place for email address I have to fix that now that I think about it, but it is a good one, and covers all the basis. Anyway if I can help email me sam-tans@shaw.ca I would be more than happy to send you the one I use, you would have to type it out and change the name etc.. but its a good one to copy.

One of the companies I have purchased solution afrom also sent me one. Don't be lured into thinking that just because something is labeled 'hypo-allergenic' someone can't have a reaction to it. It is just less likely to create a problem in the majority of people. Never thought of checking with insurance company...good idea! It never hurts to do a patch test, especially if someone has a history of reactive problems.

can anyone supply me with some names of insurance companies for liablity. I just started AB tanning. I'm guessing coverage for spray tan only will be pretty reasonable. Thanks

We also have a client wavier form. I would be interested to see what colorchange's form has. I am also more than happy to share what we use if anyone wants. Just email me and I will send it.

We have sprayed over 50,000 tans and have never had someone have an allergic reaction. But with such a litigious society in which we live, insurance is always good to have.
