Sorry, just to make sure, I would be more than happy to send you an email, with an attachment of the waiver form I use, the only thing missing is a place for email address I have to fix that now that I think about it, but it is a good one, and covers all the basis. Anyway if I can help email me sam-tans@shaw.ca I would be more than happy to send you the one I use, you would have to type it out and change the name etc.. but its a good one to copy. Goodluck |

Rep Power: 0 | One of the companies I have purchased solution afrom also sent me one. Don't be lured into thinking that just because something is labeled 'hypo-allergenic' someone can't have a reaction to it. It is just less likely to create a problem in the majority of people. Never thought of checking with insurance company...good idea! It never hurts to do a patch test, especially if someone has a history of reactive problems. |
