Employee Contracts

Does anyone have a copy of an employee contract that they use for their new employees? Would anyone be willing to share this with me? Thanks!

We have a handbook and job description that they sign.

We have an employee handbook that they must read and sign. There is a page in the back of the book that they have to sign saying that they agree with everything in the handbook.

We keep a sheet in their personal folder with emergency contact info, they initial that they have received their employee manual, tanning consultant/bed cleaner duties, a keyholder agreement form, all tax forms and a copy of their SS & drivers license.

Would it be possible do get a copy of your handbook if so is it something that could be mailed or faxed? How much would you charge to Austin, Texas, 78645.
Also I need a copy of a year contract for my customers to sign to be able to commit them to monthly charges.
