I am opening a new salon in IN and what to make sure I have all the required forms. Does anyone have sample forms that they use and would let us use also?

New Member forms?
Skin Typing Form?
Employment application?
Shift sheets?
Refund log sheets?
Bed Maintenence Log Sheets?
Salon Policies?
Cleaning check list?
Employee "Write Up" forms?
W-2 Forms?
W-4 forms?


What could ya be more specific???

Fuzzy ergoline has a nice form. We just make copies of the original one we have from years ago.

I am referring to the required legal tanning form that will hold harmless the tanning facility. If possible I would like also New Member forms, Skin Typing Form, Bed Maintenence Log Sheets, Cleaning check list. Thanks!!!

I disagree..smart tan is a waste of money..but that is my opinion.

I would go with the Looking Fit, NTTI We Get U Tan Questionaire, as a skin type analysis form, it goes into more detail than the Smart Tan Form and breaks the skin types 2 and 3 into sub-catagories to make it easier to determine session times. Also, there are quite a few different Client Release and Informed Consent forms available. You can get them from Smart Tan or NTTI and also from ITA.

I personally do not go with SmartTan because of a personal issue with them, however they are a good source of information. It costs a lot to be a member with them, but if you feel it is worth it, by all means, go for it!

I will also find the link to the Indiana Regs so that you can be sure to have all your ducks in a row for the state! It may take me a day or so, but I will get back to you!
