COULD SOMEONE PLS. EMAIL ME A COPY OF THEIR RELEASE FORMS THEY USE IN TEXAS FOR TANNING AND UNDERAGE TANNING INCLUDING SPRAY TAN.

Nothing against you but you are asking for something that cost me about $280.00. I had my attorney research and create a form for our salon.
