Waiver/Liability Release From - tanTALK

Hi,
Soon will be starting my spray tanning business (will be going mobile) I can use all the help and advice I can get to help make this a successful venture. Does anyone have a waiver or release form for clients/customers to sign to protect me from being liable in case of an allergic reaction to the tanning solution , stain to clothing, etc. (or any other things I need to be aware of. I have looked on the internet for a blanket form to adapt to my needs.......but no luck. Can anyone help? I would be very appreciative.
