Does anyone have any info on any kind of consent form for spray tans ?? I would assume as long as they are informed of the FDA report on DHA, I should be ok. I would still like something for them to sign saying that they have been informed and are aware of everyhting relating to DHA.

I would go a step further and inform them that if the solution happens to come off on their clothes or other items that you are not responsible. Definitely do it in writing.

Airbrush Tanning Release Form

I, ______________________________, release and discharge (HANDS & TANS DAY SPA) from all manners of actions, judgments, sums of money and demands whatsoever in law, in regards to Airbrush tanning.

I have been advised that the wear of airbrush tanning depends upon my skins age, texture and absorbency ability and the after care that is given. By signing this release form I release responsibility of (HANDS & TANS DAY SPA) should there be any immediate fading or loss of the airbrush design.

I affirm that I have been advised of the airbrush procedures and that the products being used are FDA approved and give my permission to proceed with the airbrush application.

I also release (HANDS & TANS DAY SPA) and their employees from any responsibility in the event I may not like the procedure. I will discuss the situation if I am not completely satisfied with the director of the facility and allow them to make arrangements for my satisfaction.

I am not allergic to any dyes, inks, nuts or cosmetics to my knowledge, except for the following______________________________.

I have been advised that I should not shower for at least 6 hours after the procedure.

I have been advised to wear loose fitting clothing after the procedure and have been told not to use harsh soaps or exfoliate when showering or bathing after this application.

I affirm that I am at least 18 years of age and in good health.

Not bad at all, but I would toss in something about staining of clothing bed linens, etc..

Hey GoldenMist...here is the one that I use...feel free to use it.

(sorry about the format .....to lazy to fix it)
PREPARATION FOR YOUR AIRBRUSH TAN

„© Exfoliate your skin with a scrub before your sunless tan. Baking soda on a scrub glove works well. Removing dead, dry skin cells will prevent uneven application of the sunless solution and improve tanning results.
„© Do not apply deodorant, perfumes, or lotions to skin prior to tanning. Shave desired areas and remove all make-up before your tan.
„© Do not wear jewellery that could cause uneven tanning.
„© Wear old, dark undergarments or a bathing suit to the session. We can provide complimentary undergarments upon request.
„© Wear loose fitting, dark clothing home from session as a small amount of ˇ§bronzerˇ¨ may rub off and be visible on light coloured clothing.


THE TANNING PROCESS

„© It takes about 4 to 6 hours for your tan to develop.
„© While your tan develops, wear loose clothing.
„© Avoid rubbing the face or other parts of the body.
„© Avoid exercising and excessive perspiration.
„© Do not shower, swim or get wet for at least 12 hours.
„© Do not sit on or wear light coloured material.
„© The initial ˇ§bronzerˇ¨ will wash off leaving your skin with a natural looking tanned hue.
„© Your tan should last 5 to 7 days depending on your skin type.
„© Keep your skin well hydrated South Seas moisturizer and South Seas tan extender. This will prolong your tan.


*For best results, we recommend 2 sessions within 7 days to achieve desired colour.
*If you have a history of Asthma, respiratory condition or allergy that may be aggravated by this
application, please consult a physician before using it.
*We will not apply an airbrush tan to pregnant women.
*You must wear appropriate undergarments. We will not apply an airbrush tan to naked individuals.




I, the undersigned, understand and will comply with all instructions and have been properly instructed prior to my Airbrush Session. I am using these services at my own risk. I hereby authorize and direct employees or agents of this service to perform such tanning procedures as may be deemed necessary or advisable. I have provided them with the information required to perform such service. I acknowledge that results of airbrush tanning do vary and that no guarantee of results is offered or implied. I hereby relieve this establishment, as well as the airbrush Technician, and hold them harmless from any liability involved in the use of the tanning process. The salons and their agents or employees are not liable for any injury to person or property or the loss or theft of any personal property.

Wow. That's pretty unfair. I'm not sure what experiences you've had, but everyone who's tried my machine has absolutely loved it. It's really natural looking, easy, and doesn't take hardly any time.
Please explain to me why you feel the way you do....

I found at www.tanningtraining.com they have a link for Sunless Buisness Kit and you can download their consent form for free. It covers all the newest info from the FDA studies on sunless.
