You can design the contract yourself. I own a gym and took contracts from 2 clubs and combined them to make my own. I ask all personal info first, name address, age etc. Then general "check Yes or No" medical conditons, then a general liability where I added that we have the right to refuse service to anyone, change operating hours etc. Try getting a hold of some from other clubs(maybe copies from friends, etc or trainers).

Toribabyg, thanks have been trying to get ahold of one but I'm in a small town and haven't been able to get ahold of one...Think I will just make a copy of my tanning one and just make some modifications....I just put in the swipe card machine so it can be open alot longer hours and I am worried about liability and how to keep up with who owes...I can cancel the card if not paid up though...Thanks Pam

Is your club going to be staffed full time or is it a key system? Keep good records. On each contract, have a spot to mark the expiry date of the membership, then have a membership record book where you mark each persons name and the date their membership expires. Check the book each day and you can see who owes you. Also have a club sign in sheet so all users must sign in or swipe their card as you suggested. How big is your gym and tanning salon?

It is a swipe card system....for the fitness center...I am only in a town of 5000 people and I have two lay down beds and 1 stand up...I bought out a physical trainer in San Antonio and have some pretty neat stuff...I also have a 42" Tv with DVD and VCR and have people that want to do the firm and palates(spelt wrong)....I also sell alot of jewelry and perfume....Need all you can get in a small town...That is my main concern is when they start and it ends...I know no matter what the contracts aren't worth the paper they are written on.....Worried about liability and theft but need people to be able to come in to work out different hours then the salon...I am putting in camera's and track lighting outside...Wish me Luck...Thanks for your help.

Pamelaha, I also own a tanning salon and a fitness center. However I would never just combine your waiver forms. I would suggest you have a attorney or contact your insurance provider and they can guide you where you need to go. The reason i would never suggest making your own or combining, etc. is your situation is unique with a swipe key. 20 years ago i would say go for it, but these days there is so many people out there who their main mission in life is to see how much money they can get from businesses these days. To protect you and your money you invested I would pay the extra money and have it done right.
