I have a gym with tanning our contract for the gym we had to get it aproved by the state. If we didn't and they found out we could get a $ 10,000 fine. We had to register the gym with the state also. Hefty fines in nj. We plan to open Dec 1st. I need a tanning waiver for my tanning clients to sign. Does anyone know where I can find one or can someone e mail me a copy of 1. Thanks so much!
