When you submit your information to DS, be sure to include a W-9 form. It says in small print at the bottom of the form that you fax that it is required. When I faxed them my info, they called me back within 10 or 15 minutes to confirm everything and I received my stuff about 5 days later. They sent TONS of samples, t-shirts, posters and a tote bag. Performance Brands also sent some samples, but not as many as Designer Skin. I didn't get responses from any other companies.
